Position Summary: Provides psychosocial services when applicable in collaboration with the case manager for patients and families in adjusting to episodes of acute and chronic illness; assists patients in coping with lifestyle changes precipitated by illness or disability. Social workers utilize a variety of treatment modalities to assist patients and families in the alleviation or resolution of social, financial and emotional problems precipitated by illness and hospitalization.
Social workers work collaboratively with the medical and nursing staff, patients, families and external agencies/resources to facilitate arrangements for post discharge care. Primary focus is to provide a holistic approach to treatment and resolution of psychosocial issues attributed by illness and hospitalization.
Position Expectations and Responsibilities:
Expected to have basic computer knowledge of HBO, STAR, Portal, Microsoft Outlook, BRCH Intranet, or the ability to learn said systems.
Expected to be able to read, analyze and interpret general business periodicals, professional journals, technical procedures of governmental regulations.
Expected to demonstrate knowledge and skills necessary to perform assessment and provide services to meet age-specific patient needs
Expected to perform assessment within 24 hours of notification from case manager, physician or multidisciplinary team by interviewing patient and/or significant other to obtain data on personal, social, medical emotional, cultural and religious history in order to delineate problems requiring social work intervention and to plan appropriate services.
Expected to participate in discharge planning rounds and health care team conferences and share pertinent data with other disciplines to determine the most effective plan of care for post hospitalization. Develops, implements and evaluates the plan in conjunction with the case manager and the multidisciplinary team.
Expected to, in collaboration with case manager, identify those barriers that result in delays in discharge and assist in the development of strategies to minimize their occurrence.
Expected to coordinate services for patients requiring: guardianship, ethics consult, psychiatric care, hospice care, out-of-area complex discharges, air ambulance services, catastrophic illness, community resources for the homeless, adoption services and long term care.
Expected to assist patients with limited resources (un-insured) in obtaining appropriate services to meet medical needs.
Expected to provide assistance to patients who report abuse and/or neglect.
Expected to provide services to women experiencing a fetal demise/stillbirth or infant loss, and patients with documented history of or current users of all illicit drugs and alcohol.
Expected to identify unmet community needs and recommend the development of new resources as indicated.
Expected to provide coverage to designated areas, units/services in the absence of the assigned social worker.
Expected to attend minimum requirement of educational in services per year for continued certification and licensure. Expected to write reports, business correspondence and procedure manuals.
Expected to be able to effectively present information and respond to questions from groups of managers, clients, customers and the general public.
Expected to involve patient/family in discharge planning and referral process.
Minimum Education and/or Experience Required:
Master's degree in social work from an accredited university AND Minimum of three years experience in a current acute care clinical setting (preferred)
Current certification as a Master's Level Social Worker or current licensure as a Licensed Clinical Social Worker (LCSW)
BLS certification from the American Heart Association
Current Driver's License, Registration, and Insurance, along with acceptable driving record
