Remote Case Manager
Job Summary: The Remote Case Manager, an office based position, collaborates with physician(s) and/or members of the interdisciplinary care team in the screening of patients and potential discharges for appropriate level of care. Identifies patients who may benefit from Case Management (e.g., high-risk, problem-prone patients). Provides information regarding community resources to patients/families. Collaborates with patients/families to develop discharge plans. Ensures that appropriate referrals to community resources (e.g. patient's financial concerns, difficult social situations, difficult placement issues, multi-disciplinary referrals) occur. Reviews the utilization of resources and works to reduce unnecessary lengths of stay and the over utilization of hospital services. Reviews the medical record for quality and utilization indicators, recording and communicating identified variances as appropriate through data collecting forms and quality indicator monitoring processes.

Education and Qualifications:
Bilingual ability a plus.
Current CPR certification.
Currently licensed to practice as a registered nurse in the State of California.
Graduate of an accredited school of nursing, BSN preferred and master's degree highly desirable.
Prefer RN with Case Management experience, or two years as discharge planner or utilization review nurse.
Three to five years of clinical experience as a RN in an acute
