Job Summary: The case manager leads a multidisciplinary team across the continuum of care to initiate, implement and coordinate all aspects of the utilization management and discharge planning function from admission through post-discharge for an assigned patient population. Primary focus is on the severity of illness of patients and intensity and appropriateness of medical services required during an inpatient stay. Additional functions include planning, implementation and evaluation of plans of care as well as ongoing concurrent monitoring of appropriateness of that care in conjunction with hospital and community resources.
Education and Qualifications:
Minimum 3 years clinical experience.
Broad clinical background in a variety of settings, and prior Utilization/Case Management experience required.
- 1-3 years previous acute hospital case management experience required.
Graduate from an accredited nursing program.
Basic computer skills in Word, Excel, and Power Point required.
Bachelor's degree in nursing preferred. Must have a current California RN license.
