HIM Clerk I

Under the general supervision of the Assistant Director of Health Information Management, the HIM Clerk I performs a variety of clerical duties to support the Health Information Management Department. Ensures that patient documentation is collected, accurately processed, retrievable and appropriately disseminated.
Essential Duties and Responsibilities
Responds to requests for medical records; understanding regulations that govern disclosure of personal health information and operating within those boundaries. 2.
Distributes physician report copies to physician offices as necessary. 3.
Rounds on medical floors processing (assembling, scanning, and linking) current patient documentation. 4.
Performs quality assurance ensuring that scanned documentation possesses necessary requirements and scans cleanly. 5.
Collects and processes (assembling, scanning, and linking) discharged charts from outpatient locations. 6.
Manages workflows and queues to ensure that documentation is archiving correctly into the electronic medical record. 7.
Collects patient documentation compiling it by discharge date and patient visit for storage. 8.
Interacts appropriately with customers, including frequent contact with clinical personnel in order to provided information.

High School Diploma or GED
Minimum Work Experience
One year clerical experience required with computer proficiency required. One year experience in Health Information Management with demonstrated knowledge of the medical record and familiarity with an electronic medical record, preferred.
- (May be met with appropriate accommodations if deemed necessary). Considerable mental concentration for sustained periods of time with attention to detail of paramount importance. Pressures of time, accuracy, and interruptions. Must handle confidential material. Long periods of sitting is required. Must be able to handle and transport 5-10 lbs. of charts for short periods of time. Standing, walking, reaching, bending, and stooping. Must be able to push/pull computerized cart to floors and stand for extended periods of time to process records. Vision must be good. Must have finger dexterity and complete use of hands and arms. Approximately 80% of working time is spent at a computer. Must have great customer service skills and the patience to deal with difficult situations.
