Oncology Case Manager, Contingent
Responsible for ensuring patients are properly admitted to the Karmanos Center Hospital (KCH) and transitioned to the appropriate level of care. Ensures patients have an effective plan of care and are receiving prescribed treatment, and advocates for services and plans patients need both during and after their stay. Oncology Case Managers concurrently plan for transitions of care, discharge and often post-discharge follow up. Will coordinate and communicate with the patient and family, physicians, funding sources (i.e. insurance, Medicare), and community resources that provide services the patient may need, such as rehabilitation facilities or providers of medical equipment. Through this coordination, Oncology Case Managers' goals are to ensure both optimal patient and hospital outcomes including quality of care, efficient resource utilization and reimbursement for services. Overall, hospital case management is a collaborative practice, consisting primarily of nurse and social work professionals working in collaboration with physicians and other members of the healthcare team.QUALIFICATIONS: Licensed to practice in the state of Michigan as Registered Nurse (RN); alternatively, current license or eligibility for licensure as a Master Social Worker (MSW) in the state of Michigan. Attained either a Bachelor's degree in Nursing or Master's degree in Social Work, along with two years of experience working in a healthcare setting either as an RN or social worker; or equivalent combination of education/experience. All appropriate licensing and/or certifications need to be current. Case management/discharge planning experience is strongly preferred. Advanced certification is preferred, such as the Accredited Case Manager™ (ACM) Certification offered by ACMA, specifically designed to validate an individual's competency in hospital case management practice. This certification is available to both nurse and social work professionals in case management. Previous experience with the oncology patients and families is strongly preferred. Excellent interpersonal and communication skills required. Strong organizational and analytical skills, along with proven ability to manage multiple assignments in fast-paced environment.
