Summary: Care Coordinator is responsible for the overall movement of patients throughout the organization in a safe and efficient manner. Coordinates care of patients through collaboration with physicians, nursing staff and other health professions. The position facilitates efficient utilization of resources, optimize clinical and financial outcomes; while promoting physician, patient and employee satisfaction. The Care Coordinator appropriately delegates patient workload and is a role model for teamwork, leadership, and professionalism.
Job Requirements: Valid Alabama RN license required. BSN preferred. At least three years hospital patient care experienced preferred. BLS required; ACLS required as determined by unit assigned.

Weekend and holiday work required
On-Call work required for after hours, weekends, and holidays
Must possess basic computer skills
Knowledgeable in use of appropriate infection control or isolation techniques when necessary
