SUMMARY OF RESPONSIBILITY - Job Summary (general statement of purpose or objective of position)The pharmacist will be responsible for a system-wide Antimicrobial Stewardship Program (ASP). The pharmacist will evaluate, recommend, design, implement, monitor, and/or modify system-wide ASP initiatives and patient-specific Infectious Disease (ID) pharmacotherapies. The pharmacist will lead, assist and/or advise the ASP Physician, Clinical Coordinators, Supervisors, and Directors of Pharmacy in system-wide ASP initiatives and antimicrobial formulary management tasks.MINIMUM REQUIREMENTS - Knowledge, skills, abilities, license, registration, certification, education, and experienceGraduate of an ACPE accredited College of Pharmacy with a Doctorate of Pharmacy degree. Completion of an ASHP accredited PGY1 pharmacy residency. Completion of an PGY2 ID pharmacy residency (preferred). Active post-licensure Pharmacotherapy certification by the Board of Pharmacy Specialties. Active Antimicrobial Stewardship Certification through the Society of Infectious Diseases Pharmacists. Twenty-four months experience as a full-time clinical pharmacist within an inpatient health-care setting. Active license in good standing to practice Pharmacy from the Arkansas Board of Pharmacy. Demonstrated and documented expertise and experience in system-wide direct & indirect patient care initiatives. Eligible and experienced in precepting APPE students from ACPE accredited Colleges of Pharmacy. Eligible to precept pharmacy residents within an ASHP accredited PGY1 pharmacy residency program.
