RN Hospice

  • Under the general supervision of the Hospice Manager, Director or designee the Registered Nurse (RN) provides direct patient care to patients using the nursing process, demonstrating clinical competence relevant to the area of nursing practice, and adhering to the ANA Code of Ethics.

    Education:

    Bachelor of Science degree in Nursing preferred.

    Graduate from an accredited school of nursing.

    Minimum Work Experience:

    One year of clinical nursing experience required.

    One or more years of Home Health nursing experience preferred.

    Certifications / Licenses

    Current RN license in the State of Missouri or Compact Licensure. Graduate nurses (GN) must obtain licensure within 90 days of graduation. Basic Life Support

    Knowledge, Skills and Abilities

    The Hospice RN may be required to serve on an agency council or committee, as a clinical coach, or in another professional role as defined by the Director. This requirement may be waived if the RN is enrolled in a bachelors or masters nursing degree program.

    Administers medications and performs planned interventions safely, competently, and efficiently by following hospital policies, procedures, and protocols.

    Assesses patient complaints of pain and reassesses pain intervention effectiveness within 90 minutes after the intervention.

    Assesses the patient's declining condition and provides rescue interventions appropriately.

    Communicates to the physician changes in the patient's condition and advocates for patient needs in a collegial manner.

    Coordinates patient care by effective collaboration with other health care team members and facilities.

    Delegate's patient care tasks to LPNs and PCAs within their scope of duties and evaluates their performance.

    Demonstrates understanding of the dynamics of health care and hospice, adapting to change in a positive and professional manner.

    Documents accurately and appropriately patient care activities at point of care including assessments, interventions, care plans, and physician communication in the patient's medical record, using electronic screens and fields appropriately.

    Evaluates the patient condition and updates the care plan accordingly.

    Follows infection control procedures and utilizes safe patient handling and movement techniques and ensures the other team members do so.

    Initiates an age-appropriate, patient-focused care plan upon admission and with patient/family involvement.

    Notifies the physician of panic lab values and other critical test results within 60 minutes of receipt from the Lab.

    Performs patient assessments upon admission and as indicated by the patient's

    Provides and coordinates patient and family education pertinent to the patient's needs, level of education, and cultural background.

    Self reflects on performance by turning mistakes and successes into learning opportunities.

    Uses critical thinking skills when assessing patient's needs, planning care, and preparing for discharge.

    Utilizes evidence-based practice, keeping knowledge and skills up to date related to clinical setting.

    Mental / Physical Requirement

    Able to assist or assume direct patient care as census directly permits.

    Left Hand: Precise Motor Function (Or Fine Manipulation)

    May assist in the supervised lifting, transporting and positioning of patients.

    Occupational Hazards - exposure to unsteady or falling patient: In the course of transferring a patient, he/she may encounter a patient who falls or loses his or her balance, causing him/her to bear the full weight of a patient or fall with him or her.

    Stand/walk 3/4 or more of shift.
