Registered Nurse (RN) Home Health / Medicare
Provides patient care as part of a multidisciplinary team based on national standards and utilizing the nursing process.
POSITION EXPECTATIONS & RESPONSIBILITIES:
Deliver skilled, professional and age-appropriate intermittent patient visits at home. Coordinate total plan of care as directed by physician and case conferences to assure continuity of care. Develop, prepare and maintain individualized patient care progress records with accuracy, timeliness and according to policies. Reassess patient at each visit and revise plan of care as appropriate. Recommend solutions for complex problems. Complete and maintain all required documentation. supervise and teach the LPN and Home Health Aide.
Minimum Education and/or Experience:
Diploma from an accredited school of nursing or Associate's degreeAND
One year of experience as a Registered Nurse
BLS certification from the American Heart Association
Current Florida Registered Nurse license in good standing
