JOB SUMMARY: The certified nursing assistant (CNA) works under the direction of a licensed nurse and is responsible for the delivery of personal care that promotes the resident's ability to function at their highest possible level. Respects each resident's rights and recognizes the individuality of each resident. Care is delivered at a level that meets or exceeds the resident's plan of care.
PRIMARY JOB RESPONSIBILITIES AND DUTIES: Demonstrates, promotes and integrates the Sisters of Providence Care Centers Mission and core values by: Listening to report, obtaining assignments and the condition of the residents, at the beginning of shift.
Providing personal care as outlined in the resident's Individual Care Plans (and care cards.) Personal care may include shower/ bath, shampoo, perineal care, oral hygiene, positioning, nails short and clean, dressing, range of motion, ambulation and insuring adaptive devices are worn (i.e., dentures, hearing aids, glasses, etc.).
Demonstrating knowledge of individual behavioral care plan. Ensuring alarms are functioning properly and in place. Conducting alarm/safety checks at each shift change and as needed. Observing residents and reporting any reactions or changes in condition to appropriate personnel by utilizing the stop and watch form. Escorting residents to and from the Dining Room for meals. Distributing and collecting meal trays as assigned on your unit. Feeding and assisting residents for meal intake as outlined by their care plans
Recording intake of solids and fluids. Distributing nourishments to residents as assigned. Reporting to Nursing if meals or nourishment are refused by the resident (ex., diabetic residents). Assisting in transporting residents to and from activities programs as required. Recording accurately on C.N.A. Flow Sheets, Bowel and weight, intake and output sheets utilizing Point Click Care. Reporting any changes in resident's base line conditions to the Charge Nurse. Responding to call bells in a timely manner. Assisting residents as able or seeking assistance to meet the residents needs. Following all safety, security, Infection Control (including Universal Precautions) and hazardous materials policies and procedures.
Performing all tasks in order to assure resident and personal safety and the protection of co-workers and facility property. Completing all required competencies. Attending all mandatory educational programs. Regularly attending a minimal of 4 staff meetings per year. Maintaining required certifications. Participating in medical transport to doctors' appointments and in escorting residents to locations outside facility or off campus grounds, i.e. (medical appointments, activities, and outings.) Ensuring that resident equipment, such as wheelchairs and lifts are clean and in good working order.
