Identifies the need for a change in the plan of care and discusses with the supervising PT; d) Effectively renders all treatments in accordance with the treatment plan and established policies and procedures; e) Demonstrates application of principles of growth and development in planning interventions appropriate to the age-specific needs of the patient. Percentage of time: 60% 2. Coordinating Treatment Schedule: a) Demonstrates flexibility and independently adjusts work schedule to accommodate specific patient/caregiver needs, and to participate in meetings and inservices without incurring overtime; b) Consistently notifies supervising PT and team members about any schedule changes in a timely manner; c) Independently reschedules missed patient treatments; d) Demonstrates initiative and flexibility assisting with coverage of department absences. Percentage of time: 10% 3. Transdisciplinary Communication: a) Thoroughly documents patient status, progress and response to treatment in objective, measurable and functional terms. Documentation is consistently timely; b) In collaboration with the supervising PT, effectively communicates status and progress. Participates in collaborative transdisciplinary treatment planning; d) Initiates timely communication with the supervising PT and team members regarding pertinent patient issues. Percentage of time: 15% 4. Discharge Planning: a) Effectively instructs patients and caregivers in home programs and functional activities; b) Actively and constructively participates in discussions regarding the discharge plan for each patient; c) Discusses with the supervising PT any patient issues that may impact the discharge plan in a timely manner. Percentage of time: 5% 5. Maintaining Clinic Environment and Equipment: a) Consistently and independently follows cleaning and equipment maintenance schedules and manages assigned inventories.; b) Demonstrates initiative in maintaining a clean, orderly clinic environment. Monitors and insures overall clinic cleanliness and organization. d) Demonstrates thorough knowledge about the various manual wheelchairs. Is knowledgeable about and independent in setting up specialized seating equipment. Percentage of time: 10% 6. Education Activities and Program Development: a) Actively participates on task forces, committees and team meetings; b) Participates in the department inservice education program c) Participates in special department projects or department/hospital quality improvement projects. LEVEL OF EDUCATION Minimum: Graduate of an accredited Physical Therapy Assistant program. CERTIFICATIONS, LICENSES, REGISTRATIONS Minimum: Licensed as a physical therapist assistant in the State of California, or eligible. Possesses a valid CA driver license. CPR certified.
